For adults seeking personal direct care. Includes unlimited visits, direct messaging, and annual wellness exam.
$100 per month.
Couple
For partners who want shared, coordinated care. Includes everything in Individual plus joint wellness planning.
$190 per month.
Family
For households of 2 adults and 2 children. Includes care coordination for the whole family.
$300 per month.
Single Parent
For 1 adult and 2 children. Full direct access to your physician and coordinated care for your kids.
$200 per month.
Additional Members
Add an extra child to a Family or Single Parent plan, or add another adult to any plan.
Additional child $50/month.
Additional adult $80/month.
